Please note: This form is not binding in any way. Filling it out is very helpful to Flying Pig Jitterbug Jamboree so we have an idea of who to expect at the event. Thank you for doing so!

You will not be charged any money for filling out this form, but if you pay early (by 4/2/09) prices are slightly discounted. You may also pay at the door of the event for a slightly higher cost. You are free to change your mind about your choices. You also may just show up at the event and pay at the door without filling out this form, so please don't hesitate to invite others to attend with you.
